Appointment Information
Please review the following in advance of your appointment:
A referral from your general practitioner (GP) is necessary. This can be posted to the surgery, faxed to us on (03) 5221 1076 or brought in person to Baycity Dermatology PRIOR to your appointment.
A referral from your GP is valid for only 12 months.
If you do NOT have a current referral, you will be unable to claim any government (Medicare) rebate for the consultation.
Non-Australian citizens are not eligible for any government (Medicare) rebate so do NOT need a referral.
Bring a list of ALL your current medications (tablets, creams, ointments, lotions, gels or sprays).
